Continuity of Care

You’re in the hospital after a hip replacement. The physician has just informed you that you need to continue your care at a nursing facility.  How will the nursing facility know about the special care you need after hip replacement, or your special diet?

Enter Menorah Manor’s answer to continuity of care, Peggy Newton, R.N.  Peggy has both her bachelors and masters degrees in nursing and more than 40 years of experience in her pocket. She is proficient in several languages – medical-speak, therapy-speak and, most important, patient –speak.

When Menorah Manor receives your referral from the hospital, Peggy visits the hospital, reviews the chart, chats with the nurses and, most importantly, with you. She becomes aware, not just of the type of surgery performed, but other chronic conditions which must be considered in your care. She reviews the physician’s orders for activity, diet, medications and any restrictions. She also tries to get to know you as a person – where you have lived, special interests or concerns, hobbies and your family.

While you are still in the hospital, Peggy returns to Menorah Manor where she begins to write a brief plan of care.  She communicates it with the people who may be involved in your care – dietitian, social worker, nursing, physical therapist, occupational therapist. She even checks with the supply room technician to be sure the items you need will be available.

So never fear that Menorah Manor won’t be prepared for your stay. When Peggy visits you in the hospital, the foundation for continuity of care is already under way.
